There are times when a patient may need records or information sent to another physician, to family members, to an insurance company, or directly to the patient for his/her personal use.

At New Mexico Ear Nose and Throat Specialists, we are fully trained in the safe and secure handling of personal health information under HIPAA regulations.

Should you need your records, please complete the form below and mail, fax or hand deliver it to our office.  We will prepare the records and contact you when the records are ready. In some cases, there may be a fee for the records. Our office will notify you if there is a fee for the records and/or mailing.

If records are to be sent to another physician or facility, please indicate the  name of the facility or physician and their complete address.

If someone other than yourself will be coming to pick up the records at our office, please indicate this on the form below. That person will need to show a photo ID before we will release the records to him/her.

If you come to the office to pick up your records, you must also bring a photo ID.  This is to protect you from fraud and to ensure that your personal health information is given only to those people and offices that you have designated.
